Python Addin Question

Hi all,

I can see my Add-Ins icon in the Add-in Manager. However, I can not see the addin icon in the "Customize". My computer is 64 bit computer and installed with ArcMap 10.1. I have spent hours in this question and am still swamped. Please let me know if you have any suggestion.
